Waiver of Liability

I have read and understand the One Family Athletics Release of Liability Waiver. By checking this box, I acknowledge the risks associated with participation and agree to the waiver terms on behalf of myself and/or my child participating in One Family Athletics programming.

ONE FAMILY ATHLETICS RELEASE OF LIABILITY WAIVER In consideration for being permitted to participate in training programs provided by One Family Athletics, I, the undersigned parent/guardian and/or participant, acknowledge and agree to the following terms: Assumption of Risk: I understand that participation in training activities, including running, speed and agility drills, plyometrics, strength and conditioning exercises, jumping, landing, games, and field or gym activities, involves inherent risks. I voluntarily assume all risks associated with participation. Release and Waiver: I release, waive, and discharge One Family Athletics, its owners, instructors, employees, coaches, volunteers, and affiliated individuals from any claims, liabilities, demands, actions, or causes of action related to injuries, damages, or losses arising from participation in One Family Athletics programming. Compliance with Rules: I agree that I and/or my child will follow all rules, safety instructions, coaching directions, and expectations provided by One Family Athletics during training sessions. Medical Authorization: In the event of an emergency, I authorize One Family Athletics staff to seek or obtain medical treatment for myself and/or my child if necessary. I understand that I am responsible for any medical expenses incurred. Medical Conditions: I understand that it is my responsibility to notify One Family Athletics of any medical conditions, injuries, allergies, limitations, or concerns that may affect participation. Knowing and Voluntary Agreement: I have read and understand this Release of Liability Waiver and agree to its terms knowingly and voluntarily. Email Communications: I agree to receive email updates and informational communications from One Family Athletics, including training updates, schedule reminders, program information, and announcements. Text Message Communications: I agree to receive recurring text messages from One Family Athletics, including training updates, schedule reminders, event announcements, and program information.
